Send your complete NSFAS application form, together with the requested supporting documents, to Unisa Student Funding, Registration Building, Hall B, Sunnyside, PO Box 392, Unisa, 003 OR deliver it to any of the Unisa regional centres (Student Administration Directorate).I declare that I have read and understood the contents of this Loan Application Form, and that the information supplied by me is true and correct. I also understand that should any of the information/documents herewith be proven to be incorrect or falsified, disciplinary action will be taken against me.
