FEDERAL DIRECT STAFFORD LOAN APPLICATION REQUEST FORM 2011-2012. STAFFORD SUBSIDIZED: Loan amount up to your need (within program limits) determined from your FAFSA need analysis report. The Federal government pays the interest on the loan as long as the student is enrolled at least half-time and for up to 6-months after the student graduates or leaves school for any reason. Loan amounts must be prorated if you are enrolled in a program containing fewer credit hours than the statutory minimim academic year, or if you are in a program that is longer than an academic year, but your final period of study is shorter than an academic year. Loan amounts must be prorated if you are enrolled in less than a full term. You have the right to cancel all or a portion of your loan. All cancellation notices must be in writing and submitted to the Financial Aid Office before the loan funds are disbursed. You are required to complete Exit Loan Counseling at www.studentloans.gov within 30 days from the time you dropped below half time (6 credits), ceased enrollment, or graduated. The National Student Loan Data System (NSDLS) provides a website for a student and parent borrowers to track and manage their federal student loans and/or grants. It is available 24 hours a day, 7 days a week. The website is www.nslds.ed.gov . You will need your federal PIN to access this information.
